****ATTENTION PARENTS****

*Please keep your child home if they have vomited or had fever within the last 24 hours.

*There are students who have immunization records that are still outstanding.  Please take care of this ASAP!!! Your child must be current, or on schedule, with their immunizations to remain at school.

*There are medication forms for over the counter medications as well as for prescription medications.  If your child needs a prescription medication during the school day, a form must be signed by the doctor and the parent before I can administer the medication.  Over-the-counter medications must be brought to school in the original container or bottle along with a medication form signed by the parent.  Forms are available at the front office or in my office. 

* If your child has a life threatening allergy, please come discuss this with me. There are forms that need to be filled out by you and signed by a physician.  

NO MEDS ARE KEPT THRU THE SUMMER. A LETTER WILL GO HOME VIA YOUR CHILD AT THE END OF THE YEAR AND PARENT/GUARDIAN MUST PICK UP MEDS IN MY OFFICE. MEDS STILL HERE AFTER THE LAST DAY OF SCHOOL WILL BE DISPOSED OF.
